Request For More Exam Attempts

If you have used up all of your attempts on an exam without passing the exam, please use this form to request more attempts or arrange assistance beyond email contacts.

CANVAS does not automatically notify the instructor when this request is completed, therefore please email the instructor to let them know that you have filled in this request. This email must specify the exam for which you are requesting more attempts.

Please note that the instructor will check your progress in ALEKS before a new attempt will be granted. 

You will also be required to submit your completed Summary Sheet for the exam (one of the questions on this request will give you a place to do this). 

Canvas will not allow you to skip any exam. You have to pass them all. We set it up this way because Physics knowledge builds on itself. You have to master some concepts before you will be prepared to understand others.
